OT Move, Do, Play Assessment Package

In 2 appointments: 1)Virtual parent interview 45 min; 2)Child and parent appointment in clinic for 45 min. to assess and then discuss impressions, goals and treatment plan.

OT Self-reg/ Social-emotional/ Sensory Assessment Package

In 3 appointments: 1)Virtual parent interview 45 min; 2)Child visit in clinic 45 min; 3)OT and parent only to discuss impressions, goals and treatment plan.

OT My Best Self Assessment Package

In 4 parts; 1)Virtual parent interview 45 min; 2)In clinic with child for 1 hour; 3)Another in clinic with child or meeting with additional supports; 4)Virtual follow up with parents for impressions, goals and treatment plan.

OT Tricky Eater Assessment

In 2 parts: 1)Child and parent in clinic 1 hour for observation and interview; 2)Virtual 20 min. with parent only for impressions, goals and treatment plan.

OT School/daycare observation-consultation Package
