Tricky Eating Assessment Package (children under 4 years old) 

This assessment package includes two parts and is billed as a package. You and your child will have an initial in-person visit to discuss and observe the current challenges with eating relating to sensory, motor, attentional, relational and/or environmental concerns. This assessment package includes a follow up phone call with parent for 30 minutes to discuss impressions and develop goals/treatment plans.

Tricky Eating Assessment Package (children over 4 years old)

This assessment package includes three parts and is billed at $130 per appointment. The first appointment is a virtual parent assessment which will be an in-depth discussion about your child's health and eating history. A food inventory will be completed. Part 2 takes place in-clinic. Your child will demonstrate eating skills including sensory responses, oral motor and fine motor skills when having a snack of various foods. Part 3 your OT will discuss findings and impressions, will work with you to develop your goals and discuss the treatment plan possibilities for building skills in both your child and their parents/caregivers.

Move, Do and Play Assessment Package

This assessment package includes three parts and is billed at $130 per appointment. This first appointment is a 45 minute comprehensive virtual parent interview. Discussion will focus on parent's observations and description of concerns. Session two takes place in-clinic where assessment will occur with your child/youth actively participating in tasks to evaluate any of the following: pre-printing, printing, keyboarding, self-help to dress, using toys, fine and gross motor skills. Session three your OT will discuss findings and impressions, will work with you to develop your goals and discuss the treatment plan possibilities for building skills in both your child and their parents/caregivers.

Self-regulation, Social-emotional, Sensory Initial Assessment Package 

This assessment includes three parts and is billed at $130 per appointment. During the first part of this assessment your OT will conduct a comprehensive parent/caregiver interview to understand your concerns, areas of strength, aspects of school and home life, allowing you to describe what you've tried, what's helped and how parent education can support the growth and changes you are seeking for your child. Session two includes an in-clinic appointment that will engage your child/youth in activities and allow further assessment examining aspects of their cognitive, emotional, sensory, physical, or behavioural development. Session three your OT will discuss findings and impressions, will work with you to develop your goals and give handouts and information about the treatment plan for building skills in both you child and their parents/caregivers.

My Best Self Assessment Package 

This is a holistic and dynamic assessment involving a total of four appointments. It allows your occupational therapist to talk with parents/caregivers, meet the child, collaborate with other core clinical and school team supports. We recognize that each child is unique and we want to honour you child's strength while understanding their skills and next steps to becoming their best self!
